Transfer Out Forms

The Transfer Out Form is for international students planning to transfer to another college or university. The completed form must be returned to the International Student Office within the semester of transfer to another school attached with your acceptance letter for review. The International Student Office is required to transfer your SEVIS records to your future school. Failure to submit a completed form to the International Student Office may result in a SEVIS termination.

Termination Form

This form is for international students planning to complete their studies at Skyline College , or when a student has decided to terminate studies and return to their home country. Prior to departure the completed form must be returned to the International Student Office within the semester of completion of studies. Failure to submit a completed form to the International Student Office may result in a SEVIS termination.

Service Request Forms

Service Request Forms should be used when a student is requesting any type of service from the international student program. Completed forms should be turned into the International Student Office.

International Student Insurance Enrollment Form

Please complete this form to purchase the San Mateo Community College sponsored health insurance plan. Payments can be made by visa/ mastercard, check, money order or cashier's check. Payments and completed forms should be turned into the International Student Office.

Health Insurance Waiver Form

Please complete this form if you are covered under a non-district health plan. Make sure you include a copy of the identification forms which verifies coverage. This information must be renewed every semester, including summer. Completed forms with required documents should be turned into the International Student Office. Please include a copy of both sides of the Health Insurance I.D. card.
