To put materials on reserve for a course, print out the form below (or download the Word version from the link below), fill out the form, and bring it to the Library Circulation Desk.

Please fill out this form as completely as possible, bring it to the Library
Circulation Desk and allow three days for processing. Each item on reserve will
be barcoded and entered into the library database. Materials will be removed from the
reserve collection at the end of each academic year. Personal copies will be
returned to the instructor.

amount of material included in the reserve system for a specific course as a
matter
of fair use should be a small proportion of the total assigned reading.
